IRTF Feedback Form

Please take a few moments to answer the following questions about your IRTF observing run. Your feedback is the most valuable information we have on the service the IRTF is providing to its community. Accolades are reported to the relevant IRTF staff and problems raised are followed up. It is also important to note whether your run produced results which might be considered for a scientific highlight in the semi-annual reports to the NASA Management and Operations Working Group.

When you are done filling out the form, submit it using the "Send Form" button at the bottom of the form. The Feedback Form will be discussed with appropriate members of the IRTF staff. Confidential comments may be emailed directly to Alan Tokunaga, the IRTF Division Chief, by telephone at (808) 956-6691 or at tokunaga@irtf.ifa.hawaii.edu.
